The ball joint has been ruled a legal shuffle-move. Pushing with an extended arm and hacking mallets already is illegal. If not in the explicit rules, then certainly in the etiquette. Back- checking, off-ball contact and aggressive play have been addressed there too. I feel like there is consensus about how we'd like the game to be, what you're talking about is enforcement.

You and I have discussed that here before. Penalty shots would be the easiest to administrate but it has it's problems. In Chicago, we've always just awarded possession to whomever called the penalty at the site of the infraction.

As to removing a player and better referee-ing: I agree that both those things should be able to happen. But, most of our "rules" are actually just etiquette. No one has empowered a body or a person to make or enforce them.

I would rather see rules administered from the court, but I understand that in really competitive games it'd be very difficult.

The other things you mentioned are legitimate complaints. You host and organize tournaments in Madison and have a voice in how nearly any contest in the Mid West (as does everyone here) is to be run. Lead by doing.
